What is a Home Medicines Review (HMR)?
A Home Medicine Review (HMR), also referred to as a Domiciliary Medication Management Review (DMMR – MBS Item 900), is an in-depth review of a patient’s medicines for those at increased risk of medication-related issues.

✅ Performed in the patient’s home by a credentialed pharmacist.
✅ Assesses medication safety, adherence, possible interactions, and ways to improve treatment effectiveness.
✅ You the GP, remain the key decision-maker regarding prescribing and ongoing patient management.
1) Health Assessments:
Patients identified during routine assessments may also benefit from a DMMR (Item 900), particularly those at higher risk of medication-related issues.
2) Chronic Disease Management (GPCCMP):
For patients with ongoing chronic conditions who are not achieving treatment goals, an HMR referral can be considered as part of the broader care plan.

Which Patients may be considered for a HMR?
Under the MBS, a patient may be considered eligible for a Domiciliary Medication Management Review (DMMR, Item 900) if, with the patient's consent, the GP assesses the patient as:
1) having a chronic medical condition or a complex medication regimen; and
2) not having their therapeutic goals met
